Honda is recalling certain 2021–2025 Acura TLX, 2023–2025 Acura MDX, and 2023–2025 Honda Pilot vehicles due to brake pedal issues, according to the U.S. Department of Transportation, National Highway Traffic Safety Administration (NHTSA).

The June 17 recall stated that the brake pedal pivot pin was not secured properly, which can cause the pedal to shift out of position. “A brake pedal that shifts out of position can prevent the driver from applying the brakes as intended, increasing the risk of a crash or injury,” according to the NHTSA.
The issue could also cause an abnormal brake pedal feeling during operation, illuminate the brake malfunction light in the instrument cluster, or cause the vehicle's brake lights to remain lit even when the brake pedal is not applied, according to Honda’s press release.
Recalled vehicles based on supplier and manufacturing records are as follows:
  • 2021-2025 ACURA TLX, produced between Oct. 23, 2020, and Feb. 28, 2025, potentially affecting more than 7,000 vehicles
  • 2023-2025 ACURA MDX, produced between May 03, 2023, and Nov. 13, 2024, potentially affecting more than 67,00 vehicles
  • 2023-2025 HONDA PILOT, produced between Dec. 16, 2022, and Nov. 06, 2024, potentially affecting more than 184,00 vehicles
In total, over 259,033 U.S. vehicles are potentially affected, although Honda estimates that only 1 percent of the recalled vehicles have the defect. American Honda has not received any confirmed reports of crashes or injuries related to this issue.
According to the NHTSA Safety Recall Report, the issue was found to have come about when the brake pedal supplier, Otsuka Koki Co., Ltd., closed their U.S. parts production facility by October 2024 and moved production to their facility in Mexico.

"As part of the production transition from the U.S. to the Mexico facility, the supplier increased staffing levels to build parts inventory and mitigate potential supply disruptions associated with the move. However, due to insufficient training, the staking process was not performed, resulting in brake pedals with unstaked pivot pins," according to the recall.

"Additionally, a malfunction in the QR code scanning system—designed to prevent process omissions—led the supplier to disable the function and continue production, allowing the issue to go undetected," stated the NHTSA Safety Recall Report.

The NHTSA stated that dealers will inspect and replace the brake pedal assembly as necessary, free of charge. Registered owners should also expect to receive a notification letter by mail, which is scheduled to be mailed on July 28, 2025.

Vehicle owners who already paid for repairs due to this issue may be eligible for reimbursement. Owners may also contact Honda's customer service at 1-888-234-2138. Honda's numbers for this recall are FLX and XLY. The NHTSA campaign number is 25V39.
