Pain is the prime reason that people seek medical care in the United States, and osteoarthritis—where joint cushioning erodes, leading to bone-on-bone rubbing—is one of the <a href="https://www.sciencedirect.com/science/article/abs/pii/S0140673621003937" target="_blank" rel="noopener">top causes</a> of that pain. As an orthopedic surgeon for top athletes and dancers, Dr. Nicholas DiNubile has spent a career mending injured joints in a bid to restore function and stave off arthritis.

Once team physician and orthopedic consultant for the Philadelphia 76ers and Pennsylvania Ballet, DiNubile joined “Vital Signs” to present his top methods for preserving joint health—from supplements to surgery.

“I’m a big believer in joint supplements. I think the data is pretty good on glucosamine and chondroitin,” he told “Vital Signs’” Brendon Fallon.

“I don't think it's going to regrow any cushions (cartilage) … but it can protect and stabilize the situation,” added DiNubile.

The now avid tennis player says he incurred his own acute knee injury playing teen football, which sparked early arthritis in the joint. In his case, he says there is no viable surgical solution.

“Early in my career, I stopped doing joint replacements. My goal was to save people's knees: Number one, through prevention; number two, through amelioration,” DiNubile said.

He likens the loss of joint cushioning that may result from an anterior cruciate ligament (ACL) tear to the potholes in the streets of Philadelphia: “They knock off a piece, and it's down to the bone, and it's like a Philly pothole … So the Philly potholes in the joint.”

This is where advances in surgical and regenerative medicine offer bone-preserving solutions where merely removing damaged cartilage was once common practice.

“Where you have a focal area of damage but the surrounding stuff's good, we can resurface that with your own cells, DiNubile said.

On “Vital Signs,” DiNubile outlines the matrix-assisted autologous chondrocyte implantation (MACI) procedure he performs, in which a person's own cells are used to regenerate new cartilage, which is then implanted in the joint.
